The Angels Initiative: striving for equal stroke care across Europe

[Music] i will talk about the angels initiative as we said and you know that one of the main goal of the european stroke organization is to implement stroke care all over europe the idea is that everybody in europe gets the same stroke care if you are admitted you cannot have let’s say a condition like a lottery so if you have a stroke in some part of europe that you will give receive a care which is not adequate based on the current knowledge for example so what we try to do with this initiative which you through learning measuring quality improvement is to shift the level of stroke care from let’s say a basic to a higher level from a high level to the highest possible level that we have and i think this is really one of the most noble initiative what is currently going on because it really affects and impacts the life of patients all over europe i focus on the european part but also there’s a worldwide program so is i will i was more involved in this part but it’s something that it’s worldwide so for example what we did is to do some um registry so we have a developer develop a rescue registry which is a quality registry where we measured the stroke activity for example in the center in 2000 let’s say 16 and then again in 2017-18 and so on and we really measured what was implemented for example we had in italy we unfortunately have not so good timing for thrombolysis we started with 70 minutes as mean door to needle time now we’re around 50 minutes 45 minutes and 30 minutes so we really could measure what we improved in other centers there was no measurement of the dysphagia and one of the quality indicator was how many patients were evaluated for this major and we could see there was a shift from the beginning and nobody really cared about this but it was um almost 100 percent of measurement then we did simulation and there was this big award program which is important because you can also show in your city to your politician how much you achieved in in your in your center to improve the the stroke treatment in your in your center what we really need to do is you know the persistency to continue as you said we have now wonderful moment we said all we did and we improved but what we need now to maintain is the standard of care for the next year and you know especially now after kovit where all the resources and all the concentration was on covet and vascular disease and no communicable disease was a little bit pushed in in an angle and we need to maintain the high level that we achieved in in in based on sensibilization of patient dedicated pathways especially with this ageless initiative everybody was talking about the dedicated pathways that this needs to be maintained and not be pushed in an angle as as we’ve had before for many years it’s important to show that it’s possible to change so the angel initiative has this angel consultant which are not doctors and they came in a certain naive way to the hospital and say oh look at your timing look what you are doing so they were not there to criticize because critique from another doctor or from another scientist is sometimes difficult to to accept so if there is something from outside saying okay i’m not here to criticize what you’re doing but i’m here to enhance what you’re doing improving what you’re doing then clearly the the the the attitude to this change and you have an involvement and it was important for the team building the ancient consultants then were there to create the networking among the different stroke teams all over europe so you can measure your frustration your success and you can say okay they did it i can do it so i think this is also teamworking and feeling that you are not alone with your difficulties is even even more more important than other things
